Insurance Plans
At the time of service, you will be given an itemized bill (superbill) which contains all the information needed by your insurance company to process your claim. Please save this document. If you are a member of a PPO/HMO in which we participate, copayments are required at time of service. Your insurance will be billed by our billing service. You are responsible for your deductible and noncovered benefits. For services rendered in the hospital, upon receipt of insurance information, we will send a completed claim.
Due to frequent changes of insurance, please call our office to verify your insurance. If your insurance plan is not listed, please contact office for up to date list of accepted insurance.
